Northern Beaches Breast Clinic welcomes referrals from General Practitioners and Specialists. A patient can attend without a referral, but will not receive a Medicare rebate towards the cost of the visit.

The Referral should include:

​

• Patient’s full name

• Patient’s full address

• Patient’s telephone contact number’s – (home, mobile)

• Patient’s date of birth

• Name of specialist to whom the patient is being referred

• Reason for referral

• Medical History

• Relevant diagnostic results and current medications

​

GP referral valid for 12 months and Specialist referral for 3 months.
